Main Purpose
- Provide outpatient and/or inpatient medical care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the patients/beneficiaries health conditions
Main Duties and Responsibilities
- Apply medical knowledge and skills to diagnose and do prevention. Carry out outpatient and inpatient consultations, prescribing the necessary treatment respecting MSF protocols.
- Keep the patient and/or his/her family informed about the illness and provide appropriate explanations about the treatment to follow, checking they have understood.
- Follow up the evolution of the hospitalized/IDP patients, through daily visits, consultations and examinations, prescribing the necessary treatment following MSF protocols, deciding whether they can be discharged or transferred to other departments or health facilities – in collaboration with other doctors- and informing their family about the patient’s evolution.
- Check and control the rational distribution of medicines and equipment under his/her responsibility and take care the quality, disinfection, and sterilization of the medical material. Assure general compliance with standing hygiene standards.
- Participate in the collection of epidemiological data and inform the line manager about any possible medical error/patient safety incident Consult the paediatrician or line manager about the patients who have a complex pathology.
- Ensure ongoing training of the medical/paramedical in multidisciplinary team in order to optimize the quality of care.
- Knows and ensures all MSF medical protocols are followed and implemented, checking universal precautions are followed at all times and reducing bio-hazard risks and improving infection control. Ensures professional confidentiality is respected.
MSF Section / Context-Specific Accountabilities
ITFC A.ER / ICU:
- To examine all new SAM & MAM cases admitted in the ER/ICU and respect the admission criteria and (triage)
- To recognize, intervene and immediately inform the pediatrician/MAM about the malnutrition children in a serious condition requiring emergency interventions.
- To refer to other NGO/Partners non malnourished children with pathologies requiring further management.
- To prescribe routine and basic treatment according to the malnutrition protocols and pediatric guidelines used in the project
- To completely and properly fill the individual card with the medical data.
- To conduct the ward rounds with nurses and nutrition aids as a team.
- To give clear instructions to the nurses on the medical care and specific monitoring of the patient.
- To frequently review patients in the ICU and conduct proper handover as a team to the next team on duty.
- To admit or move to isolation patients with contagious diseases and keep the MAM and NAM informed
Phase I, Transition And Under 6 Months:
- To daily follow the patients in the wards for medical ward round
- To estimate the medical evolution (improvement, deteriorate on etc.)
- To give the medical treatment according to the malnutrition protocol and pediatric guidelines
- To hold properly the patient folder
- To conduct the ward rounds with nurses/midwives and nutrition aids as a team
- To ensure all patients have been vaccinated before discharge.
- To do a quick late-afternoon round to be sure that the patients are stable for the night and the rounds directives are well followed.
- To prepare proper referral note for patients
Antimicrobial Stewardship (AMS) Activities:
- To request for blood culture by filling the request form correctly and completely if the patient meets the set criteria To observe the process of blood sample collection for culture
- To follow up culture results from mini lab, then discuss it with the AMS FP (Focal Person)
- To prescribe clearly the right antibiotics (dose/duration) according to the pathogen-specific guideline after discussion with the AMS FP
- To isolate patients with Multi Drug Resistant Organisms (MDRO)
- To respect all Infection Prevention and Control (IPC) guidelines when consulting a patient
ATFC / Staff Health:
- To daily follow the patients with medical problems according to the nutritional protocol.
- To prescribe basic medical treatment.
- To refer patients that meet hospitalization criteria into the ITFC
- To consult the paediatrician or MAM about the patients who have a complicated medical problem.
- To keep record of staff vaccination and give sensitization sessions in collaboration with MAM/PMR
- Proper follow up of TB patients
- To decide, in collaboration with the nurses, on the discharge of the patients.
- To conduct outpatient consultation for staff and their dependents.
- To refer to secondary or tertiary facility staff/dependents requiring higher treatment in line with the staff health policy
Others:
- Work in collaboration with the nurses and nutritional assistants to ensure quality of medical care
- Work in collaboration with the health promotion and mental health team to be able to provide a holistic model of care to the patients.
- Work as a team member, taking in consideration the non-medical problems the patients have to face (food and care problems etc.)
- Do other duties assign by line manager , which could be day shift duty or night shift day.
